Operational loop

Operational feedback loop

Findings about an output return to the process so that the current run can be corrected.

Return the output for further processing

Feedback may arise from testing, quality checks, customer observations or a control deviation. Record the finding, assign someone to correct it, and agree which step to return to. Approval is repeated after the correction.

The loop needs an exit condition

Define when quality is acceptable and the work can be handed over. If correction does not help or an agreed time limit is exceeded, escalation is needed. Resolution time and the amount of rework help track the effectiveness of corrective work.

Practical example

Testing a software release

A new release is submitted for testing. Testing reveals a critical defect, so the development team returns to implementation, fixes the issue and supplies another release. Retesting determines whether the acceptance criteria are met.

Include in your process description

  • What deviation returns the work for further processing?
  • Who corrects it, and which step does the work return to?
  • Who verifies that the correction succeeded?
  • When is the correction cycle escalated?
